SERVSAFE REGISTRATION &/or CANCELLATION RULES & POLICY

1.        Participants and/or companies must pay and register in full before attending a ServSafe Training and Examination course.
Once an individual purchases a course seat via our website’s register & Paypal payment page, he or she will be prompted to
proceed to our specific registration page after the completion of the Paypal “check out” process. All requested registration form
information must be completely filled out per class participants and submitted via button provided on registration page.

2.        If you choose to pay by check:  Participants and/or companies must sign and date invoice order form and send a copy of the
signed invoice order along with his or her personal and/or company’s check written to Nutrition & Fitness Harmony, LLC at P.O. Box
2853; Covington, GA 30015. All checks must be received at least two weeks prior to the start of the training and exam course date
you are registering-purchasing to attend.  In paying electronically: Participants and/or companies are encouraged to visit us at http:
//www.nutritionandfitnessharmony.com/Register.html and make your payment two weeks in advance prior to attending any
upcoming ServSafe Training & Examination course.

3.        ServSafe Cancellation Policy: In the event a participant cancels a course, they will not be refunded; however, they will have
the opportunity to reschedule for another class within one year of calendar date of the canceled course.  Courses are subject to be
rescheduled at the discretion of NFH, LLC at any time, in the event a course is rescheduled the participant will have the opportunity
to reschedule for another class within one year of calendar date of the canceled course.

WELLNESS 10% REWARDS PROGRAM RULES
(Non-profit organizations in Georgia, Florida, Alabama, Mississippi, North Carolina, and South Carolina)

When registering for the program, additional optional information may be requested. Participants will receive periodic special offers
and information from Nutrition & Fitness Harmony, LLC unless the participant has chosen to opt-out of receiving such offers and
information.

The Wellness 10% Rewards Program requires participants to provide the following information:
•        Contact Person
•        Organization
•        Address
•        Valid e-mail address
•        Wellness activity implemented
•        Wellness activity budget

Details of the Program

Each organization that enrolls equal to or greater than 40% of their total congregation population in the nutrition education SUPER
or GOLD “building blocks” lecture series will receive 10% of the proceeds based on registration amounts, excluding taxes and
administrative fees (Note: the organization will be responsible for taxes and administrative fees). These revenues must go
towards an ongoing wellness related program and/or an activity which will be started and continued by the benefiting nonprofit
organization at their physical site. The wellness program must be used to further benefit and empower underprivileged population
groups within the faith-based organization. A signed contract will be established between Nutrition & Fitness Harmony, LLC and the
benefiting organization which will hold the faith-based organization liable and accountable to all the stated rules and/or regulations
governing the appropriate uses for the 10% Wellness rewards program funds. Wellness related programs include, but are not
limited to the following types of activities: intense individual psychosocial therapy (which requires a licensed psychologist, a
licensed clinical social worker, a licensed psychiatrist, and/or a licensed professional counselor, etc...), medical screenings and/or
treatments performed by a medical doctor and other medical personnel at an on campus health fair, dietetic services and/or
personal fitness training or education, and/or the establishment of a exercise facility, etc...

Causes of forfeit of contract:
In the event of forfeiture of the signed contract, the benefiting organization will be required to return wellness rewards revenues
and/or funds to Nutrition & Fitness Harmony, LLC.

The 10% wellness rewards revenues are intended to provide funding towards programs established for members of the faith-
based organization to further participate in specific behavior that increases personal wellness and are not governed by rules of
chance. Nutrition & Fitness Harmony, LLC reserves the right to offer other promotional rewards to different organizations separate
from the stated 10% Wellness reward program.

Further details regarding ongoing updates to the terms and conditions of the 10% wellness rewards program are posted at www.
nutritionandfitnessharmony.com web site under our privacy policy hyperlink. Continued use of the Wellness 10% rewards program
after any such change constitutes acceptance of the change.  It is the benefiting organization’s responsibility to periodically check
the web site for possible changes to current terms and conditions governing the program.

The following statements details the primary rules governing the 10% rewards program, which may be changed periodically  in
accordance to the terms and conditions of this site located within our privacy policy. Continued use of the Wellness 10%
rewards program after any such change constitutes acceptance of the change.

1.        In order to participate in the 10% rewards program, organizations must have registered equal to or greater than 40% of their
congregation population in the nutrition education SUPER or GOLD “Building Blocks” series.

2.        Participation in the program will constitute the participant's consent to receive administrative telephone calls, mail or e-mails
relating to the participant's account status and rewards redeemed even if the participants have opted-out of receiving e-mails.

3.        Ongoing wellness related activities (as specified previously) must be carried out at said nonprofit organization’s physical site
(s) for Nutrition and Fitness Harmony, LLC education class participants to continue developing positive wellness related behaviors.

"wellness" program.

SECURITY AND FRAUD POLICY

Nutrition & Fitness Harmony, LLC actively maintains security procedures to prevent misuse or fraudulent use of the 10% Rewards
program. The rewards program is intended to reward residents of the United States.

If Nutrition & Fitness Harmony, LLC suspects that any type of fraudulent banking activity is taking place or that any of the Wellness
Rewards Program Rules are not being followed, then Nutrition & Fitness Harmony, LLC. will place the account(s) involved on hold.
After an account has been placed on hold, the account holder will be held liable for allege charge of inappropriate use of funds that
have been earmarked for specific nonprofit organizational uses.

TERMINATION OF ACCOUNT

Nutrition & Fitness Harmony, LLC shall have the right to immediately terminate a contract in the event of any conduct by a user which
Nutrition & Fitness, LLC in its sole discretion, considers to be unacceptable, or in the event of any breach by a user of the
aforementioned Terms and Conditions.

JURISDICTIONAL ISSUES

Unless otherwise specified, the Information is presented solely for, and any checks, promotions and programs are offered only to,
residents of the United States of America.  We make no representation that material on this site is appropriate or available for use in
any particular location. Those who choose to access this site do so on their own initiative and are responsible for compliance with
local laws, if and to the extent local laws are applicable.

ARBITRATION

Disputes regarding transactions effected on this site shall be submitted to Nutrition & Fitness Harmony, LLC's, within thirty days of
the event giving rise to the dispute. In the event of an error by Nutrition & Fitness Harmony, LLC with respect to the Wellness
Rewards program and/or other types of tranactions, your sole remedy shall be a credit of Rewards or other funds to your account. In
the event that any such disputes arise, the log of transactions maintained by Nutrition & Fitness Harmony, LLC will be the definitive
record as to whether or not an error took place with transactions and/or fund transfers. You agree that any contest to a decision by
Nutrition & Fitness Harmony, LLC hereunder will be submitted to arbitration under the Commercial Arbitration Rules of the
American Arbitration Association and you hereby expressly waive trail by jury. The arbitration shall be conducted in Newton County,
GA, U.S.A. in the English language by three arbitrators appointed in accordance with the Commercial Arbitration Rules. You will bear
50% of the cost of any arbitration and you alone shall be responsible for your attorney's fees. Judgment on the award rendered by
the arbitrators may be entered in any court having jurisdiction thereof. Any provision of applicable law notwithstanding, the arbitrators
shall not have authority to award punitive or exemplary damages.


GENERAL PROVISIONS

These terms and conditions shall be governed by and construed in accordance with the laws of the State of Georgia, without giving
effect to any principles of conflicts of law. You agree that any action at law or in equity relating to the arbitration provision of the
aforementioned Terms and Conditions outlined in our policy shall be filed only in the state or federal courts located in Newton
County, Georgia and/or the appropriate courts in respect of our company’s physical address and that you hereby consent and
submit to the personal jurisdiction of such courts for the purposes of litigating any such action. If any provision of these terms and
conditions shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from these terms
and shall not affect the validity and enforceability of any remaining provisions. Nutrition & Fitness Harmony, LLC's failure to insist
upon or enforce strict performance of any provision of this agreement shall not be construed as a waiver of any provision or right. No
waiver of any of these terms and conditions shall be deemed a further or continuing waiver of such term or condition or any other
term or condition. This is the entire agreement between us relating to the subject matter herein and, except as provided in these
Terms and Conditions with respect to changes to these Terms and Conditions, shall not be modified except in writing, signed by
both parties.
